HEALTH PLAN RENEWAL & OPEN ENROLLMENT INFORMATION FOR 2018-2019

Attention All Staff Who Have Medical Insurance With Maple Heights City Schools:
Open enrollment this year for anyone wishing to make changes to their insurance benefits officially begins on August 15, and runs through September 15, 2018. You will be able to begin signing up for an appointment to make changes at the convocation on August 20th. We will have a table in the back of the room for you to register if you are interested in making changes. We will then contact you to set up a time for you to discuss those changes and fill out the necessary paper work. You may also email me indicating that you are interested in changing your benefits. All benefit changes are effective October 1, 2018.
This does not include those people who may have a qualifying event, such as getting married, having a baby, a spouse losing a job, or divorce, and others you can ask about. Those events make coverage effective the date that it happens. You still need to contact me to fill out the necessary paperwork within 30 days of the qualifying event, the sooner the better. If your status change is not submitted within 30 days, benefit changes cannot be made until the next year’s open enrollment.
Finally, please take the time to read the attachments that are sent to you as required by law. There is some good information. All of these attachments also appear below.

It is open enrollment for our 2018 Flexible Spending Account plan. You can use the FSA plan to pay for your out-of-pocket healthcare or daycare expenses with pretax earnings and save. Simply designate a specific amount of your pay (before taxes are withheld) to go into your FSA to use for expenses you will have in the coming year.

Medical Costs: Family Medical $175.69 / Single Medical: $70.72
*MMO coverage for child dependents is offered until child turns 26 years old. Children can continue coverage through COBRA once not eligible on parent's plan, or seek their own insurance. Employees should inquire if this applies.

Maple Heights City Schools has contracted with a third party administrator at no cost to the District to oversee all 403B and 457 plans. The company is PlanConnect. They have been hired to monitor all transactions under the current plan and any new ones that may be added in the future by current or new employees. Their system is designed to have easy access for the employees to make changes in employee contributions, to help remit contributions to the approved providers of Maple Heights City Schools, and to assist us in assuring that our plans are compliant with all IRS regulations. Employees will receive more information directly from PlanConnect that will provide instructions to log on to their website where you can view your account information, as well as make payroll deduction changes to your 403b/457b accounts as an option to fill out paper forms.

As you may know, if you have an adult dependent child covered by the Maple Heights City Schools health and prescription drug plan, such coverage will terminate at the end of the month in which the adult child turns age 26.
