Transcripts & Records

A transcript release form must be signed by a parent/guardian or student (if over 18 years of age).  Each senior will receive a free copy upon graduation.  The first year after graduation transcripts will cost the student $2.00.  Thereafter each transcript will have a $5.00 fee.  Official transcripts will not be sent to students, parents or former students, but only to institutions or organizations designated.  Requests must be made in writing and submitted to the Community One Office, located at Maple Heights High School, 5500 Clement Drive,  Maple Heights, Ohio 44137.  Please direct questions to 216.438.6400.
