HEALTH PLAN RENEWAL & OPEN ENROLLMENT INFORMATION FOR 2020-2021
>AUGUST 15 - SEPTEMBER 15, 2020
Attention All Staff Who Have Medical Insurance With Maple Heights City Schools:
If you are a benefits-eligible employee, the open enrollment period is the time for you to make changes to your health, dental or vision coverage. You can add or delete coverage or you can add or delete eligible dependents.
- Open Enrollment is August 15 through September 15, and any changes made during this period will be effective October 1, 2020.
- If you wish to make any changes to your medical, dental, or vision benefits, please contact Terri Esber: 216-587-6100, x 3102 or email firstname.lastname@example.org.
- You are permitted to make changes to your benefits only during open enrollment, unless you experience a life change that is considered a qualifying event. (You may obtain details on qualifying events by contacting the Treasurer’s office)
This does not include those people who may have a qualifying event, such as getting married, having a baby, a spouse losing a job, or divorce, and others you can ask about. Those events make coverage effective the date that it happens. You still need to contact me to fill out the necessary paperwork within 30 days of the qualifying event, the sooner the better. If your status change is not submitted within 30 days, benefit changes cannot be made until the next year’s open enrollment.
Finally, please take the time to read the attachments that are sent to you as required by law. There is some good information. All of these attachments also appear below.
