Annual Safe Routes To School Parent Survey
Dear Parents and Guardians:
Safe Routes to School is a program aimed at encouraging students to walk and bike to school as part of a healthy lifestyle. The City of Maple Heights, Maple Heights City Schools, and the Cuyahoga County Board of Health have partnered to create this program with funding from the Ohio Department of Transportation.
Please select the link below to take a brief survey regarding your child/children’s travel method to and from school. Please identify the school or schools your child/children attend and fill out a survey for each student. The information gathered will help us improve our Safe Routes to School Programs in Maple Heights. Deadline to complete the survey is April 30, 2021.
Thank you for your participation!
